Does the witness need a stamp?
The DFA asks for the witness's official stamp on the form. If the witness does not have one, supply their business card.

FBR Witness and Photograph Requirements
Your FBR witness must be an eligible, currently practising professional who personally knows you and is not related to you. The same witness should witness the form, sign and date two photographs and certify the applicant's photo-ID copy.

Who may act as witness
The current DFA list includes police officers, teachers and lecturers, clergy, doctors, nurses and specified health professionals, pharmacists, dentists, lawyers, notaries, commissioners for oaths, bank or credit-union managers, accountants, elected representatives, vets and chartered engineers. Always check the current official list before arranging the appointment.
What the witness does
- Witnesses the applicant's signature on the printed application form.
- Signs and dates two of the four colour photographs.
- Certifies the photocopy of the applicant's current state-issued photographic ID as a true copy.
- Adds their official stamp to the form or supplies a business card if they do not have a stamp.
Common errors to avoid
- Using different people for the form, photographs and ID copy.
- Choosing a professional who does not personally know the applicant.
- Using a relative, even if that relative has an eligible profession.
- Sending original photo identification instead of the certified copy requested.
- Attaching photographs to the form when the guidance says not to attach them.
Minor applications
A person applying for a minor must also follow the extra applicant/guardian photograph requirements in the current DFA list. Because minor scenarios differ, check the exact route rather than reusing an adult checklist.
Common questions
Can a relative who is a doctor witness me?
No. The witness must not be related to you, even if they work in an eligible profession.
